1) First, order the Holy/Memorial Card design and add it to your cart.

2) Then fill out this order form. If you want to order more than one design, you must fill out ONE FORM PER DESIGN. For example, if you want cards of Divine Mercy and Our Lady of Guadalupe, you must add each design in your cart and fill out a separate form for each design.
